I agree that all submitted personal data may be processed for the following purpose (“Purpose of processing”):

Providing the Data Controller’s digital connection service between healthcare professionals and authorized holders of marketing authorizations for medicinal products and/or their authorized representatives in the Republic of Croatia (“Authorization Holders”), regulatory bodies (public authorities) maintaining registers of healthcare professionals and responsible for conducting and evaluating continuing education and issuing licenses/authorizations for independent practice, in order to enable Authorization Holders to carry out advertising and information activities directed at healthcare professionals regarding medicinal products, in accordance with applicable regulations and this Consent, as well as organizing digital workshops, courses and education for healthcare professionals by Authorization Holders and their authorized representatives in accordance with applicable regulations and this Consent, and to enable regulatory bodies (public authorities) maintaining registers of healthcare professionals and responsible for conducting and evaluating continuing education and issuing licenses/authorizations for independent practice to carry out activities of their mutual cooperation in accordance with applicable regulations and this Consent.

I agree that all submitted personal data may be forwarded for the described processing to the following Recipients/Categories of recipients: all approved and registered holders of marketing authorizations for medicinal products in the Republic of Croatia, in accordance with the official records from the digital database of the Agency for Medicinal Products and Medical Devices of the Republic of Croatia (“HALMED”), according to information available at http://www.halmed.hr/Lijekovi/Baza-lijekova/, Croatian Medical Association (OIB: 60192951611), Ministry of Health of the Republic of Croatia (OIB: 88362248492), Croatian Heart House Foundation (OIB: 52207997111), Croatian Institute of Public Health (OIB: 75297532041), regulatory bodies (public authorities) maintaining registers of healthcare professionals and responsible for conducting and evaluating continuing education and issuing licenses/authorizations for independent practice.

The submitted data will be stored by the Data Controller for the entire duration of the described digital connection service with any of the Authorization Holders for the purpose of fulfilling the Purpose of processing, and also after the termination of the described service for a maximum of 10 years from this acceptance, unless the Data Subject subsequently approves an extension of the data storage period stated herein.

I am fully aware of all my rights under applicable data protection regulations, and in particular the following rights: to request from the Data Controller at any time; access to my personal data, rectification of my personal data, erasure of my personal data, restriction of processing with respect to my personal data, exercise of the right to data portability of my personal data, or to object to the processing of my personal data. I may exercise any of the above rights directly through the Data Controller’s Data Protection Officer.

I am fully aware of my right to withdraw this Consent at any time, as well as to lodge a complaint with the supervisory regulatory authority at any time.

VII

I expressly consent that the processing of my personal data may include processing for the purpose of profiling and/or automated decision-making based on such processing.
